Dear PAS Families,
We are very excited to be opening up Session One of our Enrichment Club Program the week of September 14th.. Attached is a copy of the sign-up sheet that will need to be submitted in order for your child to participate. If you are interested in enrolling your students in this exciting and fun program, please see below.
For students not enrolled in After Care:
In order for your child to be registered to participate, both of the attached forms must be completed in full. On the second page of the Club Registration & Agreement Form is a complete list of the clubs being offered and on what days. Without this form, I do not know what clubs your student is interested in attending. The Registration Fee of $10 per child, along with the first month of tuition ($10 per club, per child, per day) must also be paid prior to attendance. If payments are made on-line, you must attach payment confirmation to the registration form.
For students enrolled in After Care:
Only the Club Registration & Agreement Form must be completed with the list of clubs your student will be attending. If a Club Agreement Form is not turned in, your student will remain in regular After Care. There are no additional fees for students already enrolled in After Care.
*Please note that some clubs have a limit on the number of students that can participate. These spots will be filled on a first come, first served basis.*
**Some clubs require special equipment or materials that must be provided by the student in order to participate (i.e., Duct Tape Club). If you have questions about the materials needed, please contact me.**
Club Registration will not be accepted after Friday September 11th. If your student is interested in participating, their paperwork and registration fee must be turned in by that date.
